{"id":419,"date":"2023-02-05T22:42:54","date_gmt":"2023-02-05T11:42:54","guid":{"rendered":"https:\/\/conssydney.esteri.it\/?page_id=419"},"modified":"2025-06-24T10:46:35","modified_gmt":"2025-06-24T00:46:35","slug":"parental-consent-form-in-case-of-minor-children","status":"publish","type":"page","link":"https:\/\/conssydney.esteri.it\/en\/servizi-consolari-e-visti\/servizi-per-il-cittadino-italiano\/passaporti-e-carte-didentita\/parental-consent-form-in-case-of-minor-children\/","title":{"rendered":"Parental consent form in case of minor children"},"content":{"rendered":"<div>\n<p><strong>PARENTAL CONSENT IN CASE OF MINOR CHILDREN<\/strong><\/p>\n<p>&nbsp;<\/p>\n<p><strong>Passport and Electronic Identity card (CIE) requested for a minor<\/strong><\/p>\n<p>If you wish to apply for an Italian passport or electronic identity card (CIE) for a child, both parents must sign the filled in passport or electronic identity card\u00a0(CIE) application form.<\/p>\n<p>If only one of the parents lodges the passport or electronic identity card\u00a0(CIE) application form for their minor child, they will have to provide the<strong> <a href=\"https:\/\/conssydney.esteri.it\/wp-content\/uploads\/2024\/06\/Atto-di-assenso_2024_06_24.pdf\">consent form<\/a> <\/strong>signed by the other parent<strong>.<\/strong>\u00a0Parental consent is mandatory whether you are single, married, separated or divorced.<\/p>\n<p>If the absent parent is an EU citizen, they can complete and sign the form and provide a photocopy of their passport or identity card (signature and photo must be clearly showed).<\/p>\n<p>If the absent parent is not an EU citizen, they must complete and sign the form in front of a Police Officer of NSW, who will authenticate their signature by signing and stamping the form and a copy of the parent\u2019s passport. If the absent non-EU parent isn\u2019t in NSW, they will have to sign the consent form in person at their nearest Italian Consulate\/Embassy or Questura (Police station in Italy).<\/p>\n<p>The consent form is valid for\u00a0<strong><u>30 days<\/u><\/strong>\u00a0from the date it has been signed.<\/p>\n<p>If the other parent is deceased, you must provide a death certificate together with your passport or electronic identity card (CIE) application form.<\/p>\n<p><strong>In case a consent form cannot be presented<\/strong><\/p>\n<p>If the other parent refuses to sign the consent from, you can apply for a Consular Decree which will allow the Consul General, serving as a Guardianship Judge, to authorise the issue of your passport (or other travel document) in exceptional circumstances.<\/p>\n<p>Your Consular Decree application must be made in writing. It should detail the reason(s) why the other parent is refusing to give you his\/her parental consent and the reason(s) why you feel the refusal is unjustified. Your application should also include detailed information and copies of relevant documents about child support payments or custody arrangements if any.
