APPLICANT

1. Appointment confirmation printout;
2. Application form, to be signed in the presence of the Consular Officer on the day of the appointment;
3. Applicant’s passport;
4. Proof of residence within this consular district (e.g., utility bills, Australian driver’s license);
5. Original birth certificate, with apostille and Italian translation;
6. Original marriage certificate, with apostille and Italian translation;
7. Where applicable, original divorce decree, with apostille and Italian translation;

ITALIAN ANCESTOR BORN IN ITALY AND EMIGRATED TO AUSTRALIA

1. Photocopy of the birth certificate issued by the Italian municipality where the ancestor was born;
2. Photocopy of the marriage certificate if the marriage took place in Italy, or original marriage certificate, with apostille and Italian translation, if the marriage was contracted in Australia;
3. Where applicable, original divorce decree, with apostille and Italian translation;
4. Certificate of non-naturalization in Australia or Australian citizenship certificate, issued by the competent authorities, demonstrating that the Italian ancestor did not acquire Australian citizenship before the direct descendant reached the age of majority
5. Photocopy of the death certificate if the death took place in Italy, or original death certificate, with apostille and Italian translation if the death took place in Australia;

DESCENDANTS BORN IN AUSTRALIA

1. Original birth certificate, with apostille and Italian translation;
2. Original marriage certificate, with apostille and Italian translation;
3. Where applicable, original divorce decree, with apostille and Italian translation;
