Lost/Stolen PASSPORT

1. 1. Report the incident to the NSW Police and take note of the report reference number;
   2. Fill out the report form for theft or loss of passport and send it by e-mail to the Consulate (info.sydney@esteri.it), together with a scanned copy of another ID document;
   3. If necessary, visit the Consulate during opening hours, to apply for a new passport and/or lodge the report in person.

Lost/Stolen ITALIAN DRIVER'S LICENCE

1. Report the incident to the NSW Police and take note of the report reference number;
2. Fill out the report form for theft or loss of documents and –if required by the Motorizzazione Civile (Italian Dept. of Motor Transport)– visit the Consular offices in person during opening hours to have signature on the report and/or own photos authenticated;

3. The duplicate of the Italian driver licence may only be issued by the Motorizzazione in Italy.

   Therefore if you need a duplicate it is advisable to contact the Motorizzazione Civile directly to verify what documents are required to obtain such duplicate. If for this purpose an authentication of signature on a delega (proxy) or authentication of photos is required, you will have to attend the Consulate during opening hours, no appointment required. As for the cost of authentications, please refer to the Consular Fees webpage. Payments can be made either via eftpos (debit card only) or
   in cash.

Lost/Stolen CARTA D'IDENTITA' (Identity Card)

1. Report the incident to the NSW Police and take note of the report reference number;
2. Fill out the report report form for theft or loss of documents and –if required by the Italian City Council (Comune)– visit the Consular offices in person during opening hours to have signature on the report and/or own photos authenticated;
3. The duplicate of the Italian Identity Card may only be issued by the Italian City Council (Comune) of residence. Therefore if you need a duplicate it is advisable to contact the City Council directly to have information on the application process. If for this purpose an authentication of signature on a delega (proxy) or authentication of photos is required, you will have to attend the Consulate during opening hours, no appointment required.    in cash.
